No, we do not require it for membership but we still recommend having insurance. Being a member of our practice covers your primary care needs but does not cover hospitalizations, specialty care, labs, imaging or cost of medications. Data has shown that having a doctor who knows you well and partners with you results in better health outcomes and a safer healthcare experience.

Yes! you will need to sign a one time waiver declaring that neither you or our office will directly bill medicare for our services. Your medicare can still be used for labs, imaging, medications and specialist care as well as hospitalizations.
Yes, there is one time sign up fee of $150 that goes into collecting and reviewing your medical records. This is included in the annual membership fee and is nonrefundable. Please keep in mind that in order for Dr Majorek to know your health needs, she needs to extensively review your medical history, medications and any other records that you may have.
We recognize that situations can change, and although it would be unfortunate to see you leave, we are here to support your transition to another physician. Regarding refunds, these will be calculated on a prorated basis, with the deduction of a $150 nonrefundable registration fee that covers the collection and physician review of your patient records. Please note that if you have completed your annual wellness exam before departing, no refund will be granted
You can choose to use your insurance for specialty care, hospitalization, cost of labs, imaging and medications. Your primary care needs, collaboration and coordination by your doctor with other specialists is all included in the annual fee.
Your doctor will take an active role in coordinating your care, working closely with your specialists to ensure you receive the most prompt and highest quality care. This collaborative approach offers you and your family unparalleled peace of mind.
No. This is the time you use your insurance - hospitalizations or surgery/specialist. We recommend that you have some sort of safety net for unfortunate unexpected serious medical conditions that may occur. In addition, your doctor will help coordinate and will collaborate with your specialists to ensure the most timely and best care. You will have your CarePartnersMD doctor by your side every step of the way! *HMO insurance plans can be challenging with concierge medicine, as we are not part of insurance plans/networks. Our services pair well with PPO plans, self insured folks or patients with no insurance.
